The following agencies have been funded for services for youth with Special Needs in Fiscal Year 2014 ― October 1, 2013 through September 30, 2014. Services may be delivered through more than one service location and/or funding Agreement.  Some services are provided in Client homes. 

Unless specified otherwise, all services are for youth with special health care needs ages 0-17 or through age 21 if enrolled in a non-postsecondary school/GED/non-postsecondary vocational school.

Services are available for children from families with incomes up to 400% of the Federal Poverty Level who do not have Medicaid, private insurance or another reimbursement source for the services.

2-1-1 Broward (First Call for Help of Broward)

  • Benefits Assistance and Benefits Workshops and other supportive services to facilitate access to public benefits for children, ages 0-17, with a chronic developmental disability attributable to a mental and/or physical impairment that results in functional impairment in three or more areas or who are at high risk for these disabilities.

Achievement & Rehabilitation Centers, Inc.

Broward Children’s Center, Inc. 

  • In-Home and Out-Of-Home Respite Care and other supportive services for children, ages 0-17, with developmental or physical disabilities and special medical conditions resulting from the disabilities or who are at high risk for these disabilities.  
  • Medical Home Program for children, ages 0-17, with developmental or physical disabilities and special medical conditions resulting from the disabilities or who are at high risk for these disabilities.  Services include Psychosocial Evaluation; Developmental, Nutritional and Special Education Assessments; Medical Social Work and Health Education.
  • Individual Advocacy services for parents of youth, ages 0-17, with a chronic disability (developmental) disability attributable to a mental and/or physical impairment that results in functional impairment in three (3) or more areas, or who are at high risk for these disabilities. 

Children’s Diagnostic & Treatment Center, Inc.

  • Medical Home Program for children, ages 0-17, with developmental or physical disabilities and special medical conditions resulting from the disabilities or who are at high risk for these disabilities.  Services include Psychosocial Evaluation; Developmental , Nutritional and Special Education Assessments; Medical Social Work and Health Education.

Jewish Adoption and Foster Care Options (JAFCO)

  • Out-of-Home Children's Respite and other supportive services for youth, ages 0-17, with a chronic developmental disability attributable to a mental and/or physical impairment that results in functional impairment in three or more areas or who are at high risk for these disabilities.

 Legal Aid Service of Broward County, Inc.    

  • Individual Advocacy services for parents of youth, ages 0-17, with a chronic developmental disability attributable to a mental and/or physical impairment that results in functional impairment in three (3) or more areas or who are at high risk for these disabilities.

Parent to Parent of Broward County, Inc. 

  • Individual Advocacy services for parents of children and youth, ages 0-17, with a chronic disability developmental  disability attributable to a mental and/or physical impairment that results in functional impairment in three (3) or more areas, or who are at high risk for these disabilities. 

United Cerebral Palsy of Broward County, Inc.

  • In-Home Respite Care and other supportive services for youth, ages 0-17, with  developmental/physical disabilities and special medical conditions.
  • Individual advocacy services for parents youth, ages 0-17, with a chronic disability developmental disability attributable to a mental and/or physical impairment that results in functional impairment in three (3) or more areas, or who are at high risk for these disabilities.
