The following agencies have been funded for services to meet Special Health Care needs in Fiscal Year 2012 ― October 1, 2011 through September 30, 2012. Services may be delivered through more than one service location and/or funding Agreement.
Unless specified otherwise, all services are for youth with special health care needs ages 0-17 or through age 21 if enrolled in an academically accredited program leading to a high school diploma or its equivalent.
Achievement & Rehabilitation Centers, Inc.
- Behavior modification, children's respite (in-home and out-of-home) and other supportive services for youth with chronic physical, developmental and behavioral conditions, who have mild to severe maladaptive behaviors.
Ann Storck Center, Inc.
Out-of-home children's respite services for ages 0-5.
Broward Children’s Center, Inc.
- Out-of-home children's respite and other supportive services.
- In-home children's respite and other supportive services.
- Medical Home Model program services including psycho-social evaluation, developmental assessment, special education assessment, nutritional assessment, health education, medical social work and other supportive services.
Children’s Diagnostic & Treatment Center, Inc.
Medical Home Model program services including psycho-social evaluation, developmental assessment, special education assessment, nutritional assessment, health education, medical social work and other supportive services.
Legal Aid Service of Broward County, Inc.
- Individual legal advocacy services.
Parent to Parent of Broward County, Inc.
- Individual advocacy services.
United Cerebral Palsy of Broward County, Inc.
In-home children's respite and other supportive services
