Blood Drive at Governmental Center, June 18 - 20 |
Community Blood Centers of Florida is hosting a blood drive throughout the County. They will be on-site in Governmental Center from June 18 - 20 by the Brickell Avenue entrance. Donors will receive goodies such as a free t-shirt and complimentary wellness checkups. Those interested in donating can actually enjoy twice the benefits.
Humana members who are currently enrolled in Humana Vitality, an online interactive wellness program, can earn 50 Vitality Points when donating. A form must be submitted along with documentation within 90 days of the donation. You can print the form below and submit either through fax or e-mail. You can also log onto Humana Vitality, under Get Rewarded tab and submit online. When filling out the form, please have your Humana card on hand to complete the form. Encourage your coworkers to donate.
