
The Open Enrollment period has ended. You must review your first pay stub of 2013 on 01/11/13 and compare it to your Open Enrollment confirmation statement. You must verify that your health, dental, vision, and/or FSA elections are deducted correctly. Waiver of Coverage - All benefit-eligible employees are required to enroll or waive coverage online. To continue to receive the Waiver Credit of $119.23 per paycheck, you must waive coverage online. Failure to waive coverage online will result in loss of the Waiver Credit.
Health - Co-pay for urgent care at Concentra facilities lowered to $30 (all other Urgent Care facility copays remain $50).
Dental - Rate reduction for The Standard Insurance Company Dental PPO plan along with enhanced benefits covering implants and porcelain fillings in back teeth.
Pharmacy - Catalyst Rx (formerly Walgreens Health Initiatives) has been purchased by Catamaran. Members will receive new ID cards in early January. Members should continue using their Walgreens ID card until that time.
Flexible Spending Accounts (FSA) - Maximum contribution for Medical Expense reduced to $2,500 per year. Maximum contribution for Dependent Care remains at $5,000 per year.
Contact Information - You can now update your contact information and emergency contact online.
All other after-tax benefit deductions such as optional life, legal, long term disability, long term care, AFLAC, etc. will rollover, where applicable, without you having to take further action.
